4) Medical Form - Either a Physical Dated in 2025 OR a Doctor's Clearance Note dated in 2025.  Please note that while Physicals are Valid for 12 months, AYFC rules mandate that the medical form be dated in 2025.  As such a physical form from fall 2024 IS NOT sufficient.
